Dr. Daniel Fleck, PT, DPT, OCS
Licensed Doctor of Physical Therapy, Daniel Fleck is an Orthopaedic Certified Specialist earned from the American Board of Physical Therapy Specialties. He is passionate about sport-specific and functional fitness, employing an evidence-based therapeutic approach utilizing various techniques to ensure excellent patient care.
He understands injury prevention and recovery from a medical perspective as well as an athlete’s. While in college, he played baseball, and was awarded the Academics All-Region Award for excellence in academics while participating in athletics. Currently, he is an enthusiastic golfer and USGA member. Pursuant of a sub-specialty in running, he has attended many continuing education symposiums on gait analysis and biomechanics. Richard Sarmiento, MPT
With over 10 years experience practicing physical therapy in the state of New York, Rich Sarmiento, MPT has helped countless men and women become stronger and pain-free. Mr. Sarmiento currently owns and practices at NYPT Health and Rehab, where he provides one-on-one care to every patient he treats in the Flatiron District of Midtown Manhattan in New York City.
After earning his bachelor’s degree from Long Island University-Brooklyn, Mr. Sarmiento completed his master’s in adult rehabilitation at Boston University. He’s currently a registered physical therapist in the state of New York.
As a physical therapist, Mr. Sarmiento’s dynamic approach to rehab emphasizes education, prevention, and personalized care. Patients seek him out for his thorough attention to detail, as well as his personable, yet professional, manner.
Mr. Sarmiento’s expertise in physical therapy encompasses a broad range of sports medicine and orthopedic conditions. Jessica Silva, PT, DPT, CFMT
Dr. Silva comes to High Performance PT with 12 years of experience in outpatient orthopedic physical therapy. She graduated magna cum laude from the University of Rhode Island with a Bachelor’s degree in Exercise Science and then went on to earn a Doctorate in Physical Therapy (DPT) with an Orthopedic concentration from Columbia University. Dr. Silva is passionate about helping her patients and frequently pursues continuing medical education to increase her knowledge and skills, as she views learning a lifelong process. She attends courses and conferences through various organizations including the HSS and NYU hospital networks, and is trained in many specialized manual techniques.
In 2017, after extensive coursework, Dr. Silva became a Certified Functional Manual Therapist (CFMT) through the Institute of Physical Art (IPA), joining less than 350 practitioners worldwide who hold this designation. Her treatment philosophy is heavily influenced by Functional Manual Therapy (FMT), utilizing the MNM approach to identify and treat dysfunctions in Mechanical capacity, Neuromuscular function, and Motor control in order to optimize her patients’ function and maximize their movement potential.
Dr. Silva believes in a whole body approach, looking for the drivers of a patient’s symptoms both locally and in adjacent joints or even other parts of the body, and understands the importance of reintegrating all of these parts back into a whole to fully restore the body to it’s most efficient state as a movement system.
Jessica Cole, PT, DPT
Dr. Jessica Cole, PT, DPT is a New York State licensed Physical Therapist who received her Doctorate in Physical Therapy at Stony Brook University. Her clinical affiliations were in various outpatient settings including orthopedics, neurology, and pediatrics, giving her the opportunity to evaluate and treat diverse patient populations and diagnoses. She is passionate about engaging her patients and optimizing each person’s care to meet his or her specific needs and goals, as well as utilizing appropriate body mechanics and injury prevention. Jessica is interested in treating musculoskeletal, orthopedic, and neurologic conditions, and she is looking forward to furthering her education and clinical skills through advanced specialization and certification.
Kirk Lawrence, PTA
After completing his studies in 1990, Registered Physical Therapist Assistant Kirk Lawrence began his dedicated career in outpatient orthopedic rehabilitation. In his earlier years, he coordinated and implemented Cybex isokinetic strength testing on semi-professional and weekend athletes alike. For many years, he added per diem rehab in skilled nursing facilities while continuing outpatient orthopedic work.
During his years in practice, Kirk has also served as a clinical instructor. A CI implements, facilitates, and evaluates learning experiences with students. He also continues to expand his knowledge of the field by attending continuing education courses, such as kinesio taping and workplace ergonomics.
A long-distance cyclist for nearly two decades, Kirk is now an avid runner. Having completed his second Brooklyn Half Marathon, Kirk is familiar with focused commitment to achieving set goals. It is with that same level of commitment that he helps patients regain their previous levels of pain-free function and strength.
In his free time, Kirk enjoys his love of photography, which he does professionally.
Stephen Proniewych, EP-C
Stephen Proniewych is a certified exercise physiologist (EP-C) through American College of Sports Medicine (ACSM). He strives to push people within their limits and beyond through functional exercise; including strength training, plyometrics, agility, interval training, posture correction, injury prevention and rehabilitation.
He participated in the National College of Lacrosse League (NCLL) and led them to a national championship in 2010. At Salisbury University, he earned his BS in exercise science while also volunteering hours at a physical therapy clinic and personal training facility. He has been working in the physical therapy and personal training field since 2010, specializing in sports injuries and strength and conditioning, ranging from top level athletes to everyday people.

Abby Rivas, EP-C
Abby is a certified exercise physiologist through the American College of Sports Medicine (ACSM) with expertise in calisthenics, running, and weightlifting, which she practices in her own personal training. With her clients, she incorporates principles of strength and conditioning, functional training for preventive injury, mobility training, interval and circuit training. She has volunteered in several physical therapy clinics and worked closely with experienced rehab professionals since 2012, and has been a personal trainer since 2015. Abby strives to teach clients how to maintain peak physical health in an enjoyable and sustainable way.
Abby has competed in several powerlifting competitions hosted by USA Powerlifting as well as one with the Women’s Strength Coalition to support the Ali Forney Center for Homeless LGBTQ Youth. Her passion for a healthy and active lifestyle was instilled at a young age by her father, a phys ed teacher, and it led her to eventually graduate with a BS in exercise science from Brooklyn College. A native New Yorker, Abby enjoys photography, travel, and drawing during her free time.

Rachel Chirichella, Exercise Specialist
Rachel Chirichella is a recent graduate from Syracuse University with a BS in Health and Exercise Science and a minor in Psychology. In her four years at Syracuse she worked as a personal trainer, swim coach and physical therapy aide. She has worked with a variety of clientele, with some looking to lose weight to post operation clients looking to build back muscle. She has a passion for making patients and clients feel good in their own body and is in the process of applying to physical therapy school in hopes of becoming a DPT.
She swam on the Syracuse Club swim team for four years and was captain for two of those years. As captain she wrote aerobic and anaerobic swim practices and dryland practices to build strength, power and flexibility outside of the water. In 2017 and 2018 she qualified and competed in College Club Swimming nationals at Georgia Tech, representing Syracuse in the 100 and 50 backstroke and 200 free relay. A commuter from the great state of New Jersey, Rachel enjoys spending time with her friends, family and her dog Jes.
